How to file a complaint: 
Verbal or written reports may be submitted to the Compliance Officer/Title IX Coordinator at any time, including outside of regular business hours. Reports can be made using the contact information provided above or by any method that ensures the Compliance Officer/Title IX Coordinator receives the report. Whenever possible, individuals are encouraged to use the Discrimination/Harassment Incident Reporting Form (see below) to document the concern.

Discrimination/Harassment Affecting Students and Staff Notification:
