**Please update the medical update form ANNUALLY via the parent portal nwlsd-sapphire.k12system.com. Also contact your school nurse during the school year if there are any changes in your childs health due to:
Recent surgery or injury
Medication: prescription or over the counter
Chronic illness: life threatening allergies, asthma, diabetes, seizures, heart/lung problems, ADD/ADHD, mental health concerns, migraines, or digestive or elimination problems.
Our nursing staff is available daily to provide students with quality care during the day to promote healthy school attendance and participation.
A health room is available for students in each building who are ill or need medical assistance. A registered nurse is on staff to assist students in grades K-12.
If a minor accident occurs in school, first aid will be administered. No treatment beyond first aid will be given. In more serious cases, the parent will be notified.
When to Stay Home
When your child is ill the night before a morning of school, please keep him/her home. Children who run a fever of 100 degrees or greater, vomit, experience diarrhea, coughing, sore throat, or other cold/flu-like symptoms are not ready for a rigorous day at school. The child needs at least 24 hours to get his/her body's resistance back. The child must be fever free (less than 100 degrees without medication) for 24 hours before returning to school. Please call your child's school office to report the absence.
PENNSYLVANIA SCHOOL IMMUNIZATION REQUIREMENTS
Please see the above link for entrance criteria and beginning of the year exclusion notification for Kindergarten, 7th, and 12th grade students.
Students in ALL grades (K-12) need the following immunizations for attendance:
- 4 doses of tetanus, diphtheria and acellular pertussis* (1 dose on or after 4th birthday)
- 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)
- 2 doses of measles, mumps and rubella**
- 3 doses of hepatitis B
- 2 doses of varicella (chickenpox) or evidence of immunity
*Usually given as DTP or DTaP or DT or Td
**Usually given as MMR
Students Entering 7th Grade:
- 1 dose of tetanus, diphtheria, acellular pertussis (Tdap)
- 1 dose of meningococcal conjugate vaccine (MCV)
Students Entering 12th Grade:
- 2nd dose of meningococcal conjugate vaccine (MCV) on or after 16 years of age
Please do not hesitate in getting your child the needed immunizations. Some students have already received the required immunizations but documentation was not received. Documentation should be forwarded to the school nurse as soon as possible for accurate recording. If you are uncertain of your child's immunization status, please contact the school nurse for verification.
If your child does not have health insurance or if your insurance does not cover immunizations, contact the Pennsylvania Department of Health.

Acceptable exemptions to the school laws for immunizations are:
- Medical reasons;
- Religious beliefs; and
- Philosophical/strong moral or ethical conviction
Documentation of any exemption must be submitted to the school and kept on file in the student's health record. If your child is exempt from immunizations, he or she may be removed from school during an outbreak.
Whenever possible, the parent/guardian should notify the school nurse in advance that the child will need medication. Medication will then be administered during school hours when the following conditions have been met:
- A "medication authorization" form is completed and signed by both the parent and ordering physician.
- All prescription medication must be provided in the original prescription container.
- Over the counter, nonprescription medication, will only require written parental permission via the Medical Update Form on the Parent Portal.
- Policy regarding the asthmatic student:
- Students in grades 6-12 may carry his/her own inhaler to use as medically necessary without supervision when a signed medication authorization form is returned signed by the parent and physician.
- Whenever possible, medication needed for more than one day should be given before or after school hours.
- Discontinuance of a medication will only occur with a doctor's order or written parent notification.
- Prescription medication must be delivered to the school by an adult and given to the nurse or main office.

Hearing tests are performed annually by school the health staff on children in Kindergarten through 3rd, 7th and 11th grades. Children with previous hearing problems in other grades will also be screened. Classroom teachers may refer children for hearing tests.
The school health staff administers vision tests to all students annually and any screening failure is reported to parents.
The school nurse will screen 7th grade students for scoliosis. The parent/guardian will be notified if a referral is necessary. If the parent/guardian chooses to not have their student screened, written documentation from the parent is required.
The Pennsylvania school health law requires physical exams for children upon original entry into a Pennsylvania school, 6th grade and 11th grade. These grades were selected because they represent critical periods of growth and development. Parents may wish to have their family physician examine the child and complete the required forms or have the child examined by the school doctor.

The Pennsylvania school health law requires dental exams for children upon original entry into a Pennsylvania school, 3rd grade, and 7th grade. Parents may choose whether to have their child examined by the school dentist or family dentist and complete the required forms.
Parental permission will be obtained for a school dental exam. The dentist only does a screening, a referral is sent if the child needs dental care. The school dentist does not provide dental repair.

Height/Weight/BMI (Body Mass Index)
Pennsylvania state requirements mandate all school students be screened yearly for height, weight, and BMI (Body Mass Index).
Those students who fall outside the state parameters will be encouraged to contact their family physician for a follow up evaluation.
